Care Coordination Center: We’re Here For You
Providing superior care and superior services to each patient and family doesn’t just happen in our hospice houses or during visits wherever our patients call home.
At Ohio’s Hospice, care is available when and where it’s needed.

It starts with our Care Coordination Center (CCC) in Dayton, which opened a few years ago. While it houses several other departments, much of the CCC is dedicated to Ohio’s Hospice Real-Time Care™.
Through Care Router, an enhanced technology system that provides visibility to patient and staff needs, we provide Real-Time Care, supporting direct patient care teams that provide clinical care 24/7 around the state.
This often allows the team to begin the initial hospice care conversation with patients and families, and to communicate better with families who may be out of the area.
With Netsmart integration, we increase our Speed-to-Care™. Nursing staff are available to support patient care needs with every call. They are able to see staff availability and work to schedule an admission nurse to the bedside within a goal of three hours.
After hours support is provided by our Crisis Care and Extended Hours teams. The CCC Triage Team staff is comprised of primarily LPNs and RNs and a patient service coordinator. The Triage staff also processes referrals for palliative and hospice care services across the state.
Innovative Care Solutions, which provides durable medical equipment and transport is also coordinated in the CCC for current and newly referred patients.
The referral management team receives phone calls from across the state. They have a real-time view to know the number of referrals for the day as well as referred patients who need to be scheduled for a visit.
Through ongoing collaboration with all of our locations, we are answering calls across the state and are able to see where staff is available to assign visits based on skill and availability in any of our coverage areas.
