Celebrating Our Nurses During National Nurses Month 

May is National Nurse’s Month. Throughout the month, we are celebrating and recognizing our nurses! Our nurses have a heart of service and a passion for caring for others who are facing some of the most difficult challenges in life. 

Nurses at Ohio’s Hospice are an integral part of our care teams. They serve in a variety of roles no matter the circumstances, complexity, the time of day, the challenges, or the miles apart. Our nurses have a heart of service and a passion for caring for others who are facing some of the most difficult challenges in life. We’re grateful to them for the difference they make in the lives of our patients and families each and every day.

At Ohio’s Hospice, our nurses show our patients and families compassion, kindness and support. They serve as key members of the care team, providing compassionate care alongside our board-certified hospice and palliative care physicians, personal care specialists (state tested nursing assistants), social workers, chaplains, bereavement counseling professionals, and volunteers.  

Throughout the state of Ohio, our nurses serve patients and families through various roles, from the admission and referral process to the day-to-day care provided wherever a patient calls home, whether that is a private residence, an extended care facility, and inpatient care center, or one of our Hospice Houses.  

Admissions and Referrals Nurses – RN Admissions Care Liaison 

These nurses are responsible for the admission process. They educate the patient and family about what hospice can provide, and they prepare all necessary paperwork to facilitate the transfer to hospice care. Admissions are completed in the patient’s home, hospitals or in nursing facilities within our service area where the patient resides.  

RN Care Manager, Extended Hours Team 

The extended hours nurse care manager visits patients in their home or nursing facility. They are responsible for routine and emergent visits scheduled during the evenings, nights and weekends. They provide education and support as needed. They ensure timely and seamless care to all of our patients. 

RN Care Managers 

These nurses are responsible for the daily care of the patient on an ongoing basis, whether that is in the patient’s home or at another care facility. They establish the comprehensive plan of care in collaboration with other members of the care team, the patient and the family.  

RN, Hospice House/Inpatient Care Center 

Nurses at our Hospice Houses and inpatient care centers provide care to patients who need on-site pain and symptom management. These nurses provided care at our Hospice Houses and inpatient care centers located across the state of Ohio.  

RN Call Center 

Nurses at our Care Coordination Center take phone calls from patients or their caregivers. They begin the process of assessment and treatment over the phone. They may give the patient/caregiver instructions for symptom management or medical treatment, consult with the hospice physician, and notify the care manager or extended hours nurse of the need for a visit.  

We’re grateful to our nurses who are available around the clock to provide critical care to those who need them most, whenever they need care, in whatever setting they call home. We thank our nurses for all they do to deliver superior care and superior services wherever we are privileged to serve, now and always.
