Join Our Volunteer Team

We are excited that you have decided to join our volunteer team and serve our patients and their loved ones. The Volunteer Services Department is available to assist you in this five-step process. Ohio’s Hospice requires that all volunteers providing direct patient care be at least 18 years of age. There are no age restrictions for community service, administrative support and non-direct patient volunteers.

Step One: Fill out a volunteer application.

Step Two: Fill out a volunteer reference form.

Click here to download the volunteer reference form. Fill out the form with two non-related references. Email the form to Info@OhiosHospice.org or mail to 7575 Paragon Rd., Dayton, OH 45459.

Step Three: Complete a Criminal Background Check.

A criminal background check must be completed for anyone 18 years of age and older.

  • No cost to the volunteer.
  • Must receive results stating “no criminal history” prior to attending training.
  • When a volunteer who is a minor turns 18 he/she becomes eligible to perform direct patient care volunteer duties and at that time a criminal background check would be required.

Step Four: Complete a 2-Step TB Test.

ALL new volunteers must complete TB testing.

  • TB tests will be administered by Ohio’s Hospice, free of charge.
  • If you have had a TB test done in the past 12 months and can provide proof of TB screening or PPD, it will not be necessary for you to have a second TB test.
  • If you cannot take the Mantoux skin test, you will need to provide a copy of your X-ray results. Ohio’s Hospice does not reimburse for X-rays.

Step Five: Register and attend volunteer orientation and training.

Our Volunteer Services Department will provide orientation dates.
